- Original Will, if any.
- Certified copy of death certificate.
- Complete marital status of decedent, including number of times
married and full name of spouse(s), including maiden names; information
regarding how marriage ended, including place, date, court and case number
of divorce.
- Name, address, telephone number, social security number and date of
birth of current spouse.
- Names, addresses, social security numbers, dates of birth, complete
marital status of all children; if a child is predeceased, similar
information for his/her children.
- If no children, above information for parents and siblings.
- Copy of titles to any property owned (land, vehicles, boats).
- List of all assets (checking account, savings account, stocks,
bonds, CDs, annuities, retirement plans, bank boxes), including statement
with account number from month of death.
- List of all debts at date of death and all debts of decedent paid
after death, including supporting documentation (mortgages, car notes,
credit card bills, utilities, medical bills).
- Copy of funeral bill and all related expenses.
